There's a lot of information here on the subject:

If you need the medical POA, you can find that info here:

I have my bro's power of attorney had his lawyer draw it up and he signed it in the court room before he went to jail.  Think I paid $700.00? but that lawyer was a thief.  But it sure does make my life easier having it.

He could get it notarized for free at his institution.   I did, and yes it has made our lives much easier.

Does anyone have the forms needed for POA in Illinois?  I am just doing this so that we can cash his last paycheck.

You may want to check out this website.  They have documents there but they do come at a price.

He just has to go to the law library and fill out the paperwork, it's free for him.  Put in for it to be notarized free, get some copies made and send the original to you.  It took my husband less than a week to get it done, and most of that was waiting for the notary.  It's not a big problem at all.  It's much easier for him to take care of all of it for you, and you don't need to drive yourself nuts over it.  You've got enough to worry about, let him take care of some things for you sweetie.  I know it made my husband feel a lot better to be helping me, they feel so disconnected in there...

This is what my POA states which the lawyer drew up for us and like KP18437 stated it's easy for them to have it notarized in the law library it's more of the wait for the notary to be present then anything else.

I hope this helps:

Power of Attorney
I, (name of inmate & #) , originally from (City, State and County) , currently detained under the Illinois Department of Corrections, hereby appoint (name of the POA) , a sample of whose signature appears below, as my true and lawful agent and attorney, and I hereby grant and give to my attorney, (name of the POA) , for me and in my name the power and authority to sign, delegate, and negotiate any and all documents of any kind on my behalf. This document will expire automatically upon my release from the Illinois Department of Corrections on (Date of release).

Speciment signature of my attorney:

______________________________
( name of POA)
IN WITNESS WHEREOF, I hereby certify to the correctness of the foregoing signature and have set my hand and seal to the foregoing power of attorney this ___ day of ___________, 20 ___.

                                                                       ____________________________________
                                                                       (name of inmate and #)

Witnessed:

Best for you to send him a few copies just incase he will have the extra.

For example...My husbands signture is just a upper case letter and basically a line after and the Notary did not accept it  wc4 he wanted my husband to write out his whole name in cursive so luckily I had sent my husband an extra copy so he was able to take care of it there and not wait for me to resend him the POA.

Thanks!  I am going to let him handle it.  I think you are right, I have enough going on holding down everything else.  It will make him feel really good to be the one  to take care of something as he already feels bad asking me to do so much.

Yeap!  Same here it took my husband no time to get it to me.  I 've had it for years now, and it is much easier if he does it.
